Birth Certificate Application

  INSTRUCTIONS TO APPLY FOR A BIRTH CERTIFICATE IN PERSON AT THE TOWN CLERK'S OFFICE OR THROUGH THE US POSTAL OFFICE, BY MAIL.To apply for a Birth Certificate at the Town Clerk's Office you must provide the following:​ 

  1. PROOF OF IDENTIFICATION IS REQUIRED AND MUST BE ONE OF THE FOLLOWING:

  • VALID DRIVERS LICENSE
  • VALID DMV DRIVERS PERMIT
  • VALID NON-DRIVERS ID 
  • VALID PASSPORT
  • VALID NY STATE BENEFIT CARD WITH PHOTO

WHO MAY APPLY FOR A BIRTH CERTIFICATE

  1. Child listed on Birth Certificate
  2. Parent listed on Birth Certificate
  3. Legal Guardian (Must provide proof) 

COST

Copies of Certified Birth Certificate are $10.00 each.   (Note: If applying for a Birth Certificate copy through the US Postal Mail, only Money Order's or Cashier's Checks will be accepted).Your Money Order or Cashier's check must be made payable to the Sodus Town Clerk. 


BIRTH APPLICATION 

Please find attached the Birth Certificate Application that must be filled out completely if applying in person or through the mail. No Birth Certificate Application will be processed if the information of the child on the certificate does not match the Birth Record filed in the Town Clerk's Office.


   IF APPLYING FOR BIRTH CERTIFICATE BY US POSTAL MAIL: 

PLEASE INCLUDE YOUR  PHONE NUMBER ON YOUR APPLICATION. IF A PHONE NUMBER IS NOT INCLUDED, IT MAY DELAY YOUR REQUEST.Once your application is filled out, you will need to wait and sign the application and present it in front of a NOTARY to get the application notarized.  (Please note, this is a NY State Department of Health Form, they do not have a section for a Notary to sign. Please have the Notary place notary stamp on side of application or where they can find a spot to sign and stamp). Do not sign your application until you are in front of the Notary.

Sodus Town Clerk, 14-16 Mill St. Sodus, NY 14551.  If you want to mail out this application through the US Postal Office, you must include a self addressed stamped envelope to where we are returning the birth certificate to. Depending on the mail, is dependent on how long the certified certificate will take to get to you. When we receive the application we usually process all requests with a two day turn around time. The remainder of time is dependent on the US Postal Office.


   EXPEDITE BIRTH CERTIFICATE APPLICATION: 

If you want to expedite your request, you will need to overnight your application to the address above.  You will also need to include a pre-paid overnight envelope with the address where we are returning the birth certificate too.  We only accept US POSTAL OFFICE pre-paid overnight envelopes to return your certified certificate.   Please take your application to the US POSTAL OFFICE and mail it overnight along with your prepaid overnight envelope, money order or cashiers check made Payable to the Sodus Town Clerk. This is the quickest way to get a copy of the certified birth certificate.    Note: If your application does not match the information on your birth certificate exactly, the application will be rejected until the correct information is listed.  ​

Death Certificate Application

 

INSTRUCTIONS TO APPLY FOR A DEATH CERTIFICATE IN PERSON AT THE TOWN CLERK'S OFFICE OR THROUGH THE US POSTAL OFFICE, BY MAIL.

To apply for a Death Certificate at the Town Clerk's Office you must provide the following:

  1. PROOF OF IDENTIFICATION IS REQUIRED AND MUST BE ONE OF THE FOLLOWING:

  • VALID DRIVERS LICENSE
  • VALID DMV DRIVERS PERMIT
  • VALID NON-DRIVERS ID

WHO MAY APPLY FOR A DEATH CERTIFICATE

  1. Spouse
  2. Siblings
  3. Parents of the deceased
  4. Lawful Representative of Deceased

Genealogy Application

TOWN OF SODUS GENEALOGY RECORDS ON FILE

1847 – 1850  we have

1851 – 1881 are missing
1882 – present 

Genealogy records are $11.00 for each record found and must be paid at the Town Clerk's Office or by money order through the mail. 

Money Orders need to be payable to: Sodus Town Clerk 14-16 Mill St. Sodus, NY  14551.

Note: 

Birth Records for Genealogy must be at least 75 years old

Death Records for Genealogy must be at least 50 years old

Marriage Records for Genealogy must be at least 50 years old
